Even better: if there's an unclimbable mountain or a lake on the map, so there's an entire area that you can't explore by design, so that part of the map will remain forever blank.

    *Far Cry 2 flashbacks*

...Unless there was a helicopter or something that I missed because I got bored half way through - just like every other Far Cry game.

Or unless you find exploits to get you there, when your not supposed to.

Witcher 3 had quite a few of these. I remember using constant jumps to get me into areas I wasn't supposed to be able to get into. World of Warcraft had a similar one, where you could access the world tree zone long before they opened it up in the Cataclysm expansion. Required going to a very specific spot and jumping to another specific spot, then you were in. Skyrim had a beneficial bug of the player being able to climb what should have been un-climbable mountains if jumped at a certain angle.

If there are allied characters (or any characters, in a game where I'm playing non-lethally) who are inevitably killed as part of the game flow, I will try my best to keep them alive.
The number of reloads it takes to keep Duncan being a survivor on Hard is ridiculous, and let's not even mention that NSF-UNATCO street fight in NYC...

Duncan from Dark Messiah? Dang, I didn't know you could keep him alive; one time I got him through the initial ghoul attack only to have a necromancer punk him with a guided flame bolt. Does he appear anywhere later or anything if he stays alive? Even a lampshade?

I can't even imagine keeping everyone alive in the street fight. I usually go for the "UNATCO guards love you" trigger, which is damn hard to get (and I'm not even really sure what causes it, but "Slaughter lots of NSF" seems to usually do the trick). I once tried tossing in those gas grenades you get just beforehand, which let me KO the NSF adequately but then the UNATCO guards turned on me for hitting them with the gas (mostly on account of them running into it)...
